Athletic Card Online Information

All KHS and MSK athletes (6th-12th graders) will need to fill out the forms below.

You will need the following information before you start:
1. Your students 5 digit Power School ID
2. Date/Month of their last Tetanus (TD) booster
3. Your Insurance Company, Policy Number and Name on Policy

1. First go to Download and Print.

a. If your child is a 6th-8th, 9th or 11th grader they will need an updated physical. The Medical Release Form is all you need to get signed when you get your child’s physical. If your athlete has had a physical after January 1, 2026, please have your physician sign off on this form or have a signed note from the physician’s office stating that the student is cleared to participate in athletics.
b. Physicals are valid for 2 years from the date of the exam.
c. Read the Athletic Handbook and the Heads up on Concussions information.

2. Second go to Electronic Participation Form

a. Fill in all areas on the Athletic Card.
b. Both you and your student will need to sign off that you read the Athletic Handbook and the Heads Up on Concussions information.
c. When you finish the athletic card it will allow you to go directly to the Concussion Management page. Please read the policy and then sign off.
3. You have now completed all online forms. If your student is a 6th - 8th, 9th or 11th grader or has an expiring physical you will need to have them bring their physical form to the athletic office or fax it to 207-985-1355.
